The nurse must hold an active license in at least one of the following states: Texas (TX), Arizona (AZ), Nevada (NV), California (CA), or Utah (UT).

Key Responsibilities:

  1. Perform clinical triage using the ClearTriage tool (training will be provided).
  2. Handle and process incoming calls from hospitals requesting assistance with discharge planning to a skilled nursing facility (SNF) or home health services.
