Assists Patients in the following: Prescription, Visit Follow Up, Non-Urgent Medical, and Test Result Questions
Review the in-basket continuously throughout the day to triage all unread messages
Reach out to patient by telephone for urgent issues (guidelines attached) and with the local site via telephone or email as appropriate
Perform appropriate chart review and respond directly to patient through Epic MyChart
Route messages requiring further clinical assistance from the site to the appropriate pool or provider (e.g., requests for changes in medication, new onset medical conditions or symptoms, referrals)
Process medication refill requests and route to provider
Document appropriately in the Epic EHR through the MyChart encounter
Review and complete if appropriate or route to local site administrative pool
Document appropriately in the Epic EHR
Qualifications:
Must be a US Registered Nurse – New York Licensed
Graduate of BS Nursing with active Philippines Registered Nurse
With Clinical or Hospital experience of at least 2 years
Amenable to work on night shift
Ability to demonstrate critical and independent thinking
Comfortable with providing patient education related to chronic conditions such as but not limited to diabetes, hypertension, asthma, COPD and heart failure,
Excellent communication skills – written, interpersonal and the ability to communicate effectively via telephone and electronically
Strong computer skills including Microsoft Office suite and Electronic Medical Records
