Responsibilities:

  • Log in to electronic health records (EHR) and the RPM system to access assigned patient charts.
  • Review monthly patient assignments, ensuring accurate and timely monitoring.
  • Check patient data, such as blood pressure readings and other vitals, for consistency and compliance.
  • Calculate and document monitoring minutes based on chart review activities.
  • Complete patient questionnaires within the system to support coding and compliance requirements.
  • Identify gaps in patient monitoring (e.g., missing vitals from CNAs or caregivers) and escalate as needed.
  • Repeat review and documentation process for each patient assigned.
  • Maintain accuracy, security, and confidentiality of all patient records.

Qualifications:

  • Must have a valid TEXAS USRN license.
  • Background in healthcare, nursing, medical assisting, or related field preferred.
  • Experience with Electronic Health Records (EHR) systems or clinical documentation.
  • Strong attention to detail for reviewing patient charts and identifying missing data.
  • Ability to calculate and track monitoring minutes accurately.
  • Familiarity with vital signs (e.g., blood pressure, heart rate, glucose) and clinical terminology.
  • Comfortable working with monthly assignments and repetitive workflows.
  • Strong organizational and time management skills to handle multiple patient charts efficiently.
  • Effective written and verbal communication skills for documentation and escalation.
  • Commitment to patient confidentiality and HIPAA compliance.
