Responsibilities:
- Log in to electronic health records (EHR) and the RPM system to access assigned patient charts.
- Review monthly patient assignments, ensuring accurate and timely monitoring.
- Check patient data, such as blood pressure readings and other vitals, for consistency and compliance.
- Calculate and document monitoring minutes based on chart review activities.
- Complete patient questionnaires within the system to support coding and compliance requirements.
- Identify gaps in patient monitoring (e.g., missing vitals from CNAs or caregivers) and escalate as needed.
- Repeat review and documentation process for each patient assigned.
- Maintain accuracy, security, and confidentiality of all patient records.
Qualifications:
- Must have a valid TEXAS USRN license.
- Background in healthcare, nursing, medical assisting, or related field preferred.
- Experience with Electronic Health Records (EHR) systems or clinical documentation.
- Strong attention to detail for reviewing patient charts and identifying missing data.
- Ability to calculate and track monitoring minutes accurately.
- Familiarity with vital signs (e.g., blood pressure, heart rate, glucose) and clinical terminology.
- Comfortable working with monthly assignments and repetitive workflows.
- Strong organizational and time management skills to handle multiple patient charts efficiently.
- Effective written and verbal communication skills for documentation and escalation.
