JOB SUMMARY/OVERVIEW: (summarize purpose/overview of this job):

The Triage Supervisor, RN is a dual-role clinical leader responsible for providing direct triage nursing services while overseeing the development, standardization, and daily operations of the organization's triage program. Serving approximately 75% in direct patient care and 25% in leadership activities, this position functions as a clinical expert and resource for staff and providers, ensuring safe, timely, and patient-centered care through evidence-based triage practices. The role leads the development of triage protocols, staff training and competency programs, quality improvement initiatives, and the expansion of triage services across all three counties to promote consistent access, clinical excellence, and operational efficiency throughout the organization.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING (state required or preferred)

  • Education:

Accredited RN program or higher required.

Current unencumbered California Registered Nurse license required.

Current BLS/CPR certification required.

  • Experience/Lived Experience:

Managerial Experience Preferred

Previous RN work experience in a triage setting preferred.

  • Special Skills/Training:

Strong Analytical And Problem-solving Skills Are Required.

Strong interpersonal and customer service skills required; must have the ability to work with a diverse population.

Strong verbal and written communication skills; Bilingual (English/Spanish) language preferred.

Strong computer and data entry skills required; electronic health record system preferred.

Responsibilities

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES (add or delete if necessary)

Provide telephone, electronic, and in-person triage services, utilizing clinical judgment and evidence-based protocols to assess patient needs and determine appropriate levels of care

Serve as a clinical resource and subject matter expert for triage staff, providers, and interdisciplinary care teams.

Coordinate patient care, facilitate communication among providers and care teams, and support continuity of care across the organization.

Deliver patient education and health coaching related to treatment plans, medications, preventive services, and chronic disease management.

Ensure accurate, timely, and compliant documentation of all triage activities within the electronic health record.

Provide day-to-day oversight of triage operations, including staffing support, workflow management, service coverage, and performance monitoring.

Develop, implement, and maintain standardized triage protocols, workflows, clinical guidelines, and best practices to ensure consistency, quality, and regulatory compliance.

Lead training, onboarding, competency validation, and ongoing professional development programs for triage staff.

Drive the expansion and standardization of triage services across all counties, improving patient access, operational efficiency, and consistency of care delivery.

Collaborate with clinical and operational leadership to identify opportunities for quality improvement, patient safety initiatives, workflow optimization, and enhance patient experience.

Monitor key performance indicators, patient outcomes, response times, documentation quality, and satisfaction metrics, implementing corrective actions and improvement plans as needed.

Promote a culture of clinical excellence, accountability, collaboration, continuous improvement, and patient-centered care while supporting organizational goals and strategic initiatives.

Performs other duties as assigned.
