Job Description

We are seeking a dedicated, detail-oriented, and compassionate Bilingual Medical Assistant / Care Coordinator to join our growing primary care team. This role plays a critical part in ensuring continuity of patient care by supporting providers with clinical administrative tasks, coordinating follow-up care, maintaining accurate patient records, and engaging patients between office visits.

The ideal candidate has at least 3 years of experience in a primary care or outpatient healthcare setting, is highly organized, and possesses excellent documentation skills. Fluency in both English and Spanish is preferred to effectively communicate with our diverse patient population.

Key Responsibilities

  • Update and maintain patient care plans within the Electronic Medical Record (EMR), ensuring documentation is accurate, complete, and aligned with provider recommendations.
  • Prepare patient charts prior to appointments by reviewing previous visit notes, outstanding orders, laboratory results, medications, referrals, and care plan items.
  • Ensure providers have complete and organized patient information to facilitate efficient and productive patient visits.
  • Accurately document visit updates, follow-up actions, referral status, and patient communications in the EMR.
  • Check patients out following appointments and schedule follow-up visits according to provider recommendations and established care plans.
  • Identify patients who qualify for Chronic Care Management (CCM) services, flag eligible patients for provider review, and assist with CCM enrollment and documentation.
  • Conduct patient outreach for appointment reminders, preventive care follow-ups, care gap closure, referral coordination, and CCM check-in calls.
  • Monitor outstanding referrals, diagnostic testing, and follow-up appointments to ensure continuity of care.
  • Coordinate with providers, nurses, medical assistants, and administrative staff to promote efficient patient flow and high-quality patient care.
  • Maintain confidentiality and compliance with HIPAA and all applicable healthcare regulations.
  • Assist with additional clinical administrative and care coordination duties as assigned.

Qualifications

  • Minimum of 3 years of experience as a Medical Assistant, Care Coordinator, or similar healthcare support role in a primary care or outpatient setting.
  • Bilingual in English and Spanish preferred, with strong verbal and written communication skills.
  • Experience working with Electronic Medical Record (EMR) systems.
  • Strong understanding of primary care workflows, preventive care, referrals, and patient follow-up processes.
  • Familiarity with Chronic Care Management (CCM) programs and care coordination is highly preferred.
  • Excellent organizational skills with exceptional attention to detail and documentation accuracy.
  • Ability to prioritize multiple tasks in a fast-paced healthcare environment.
  • Strong interpersonal skills with a patient-centered approach.
  • Ability to work independently while collaborating effectively within a multidisciplinary healthcare team.
  • Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), or equivalent certification preferred.
  • Experience supporting value-based care initiatives and population health programs.
  • Knowledge of Medicare guidelines and preventive care workflows.
  • Previous experience with patient outreach, care gap management, and chronic disease management programs.
