Job Description

Position Summary:

The Remote Patient Monitoring Care Navigator supports patients enrolled in the RPM program by reviewing and responding to out-of-range vital sign alerts, completing routine monthly wellness outreach, coordinating with clinic staff, and maintaining accurate documentation.

This role requires strong communication, organization, and follow-through. The Care Navigator will primarily monitor blood pressure and heart rate alerts, contact patients to assess how they are feeling, and escalate concerns according to established clinical workflows.

Key Responsibilities:

  • Review and respond to out-of-range blood pressure and heart rate alerts in a timely manner.
  • Contact patients by phone to assess symptoms, confirm readings, and determine whether additional follow-up is needed.
  • Escalate urgent or clinically concerning findings to the appropriate clinical team according to established protocols.
  • Conduct monthly wellness check calls with patients enrolled in the RPM program.
  • Provide patients with basic program support and help address non-clinical questions related to RPM participation.
  • Document all patient outreach, phone calls, vital sign alerts, follow-up actions, and escalations accurately in the Amana time-monitoring platform and in the clinic's Athena electronic health records.
  • Communicate with clinic Practice Operations Managers, medical assistants, and other care team members regarding patient questions, concerns, and follow-up needs.
  • Track unresolved concerns and ensure appropriate follow-through with patients and clinic staff.
  • Maintain patient confidentiality and comply with HIPAA, organizational policies, and documentation standards.
  • Support patient engagement and encourage consistent participation in remote monitoring.

Qualifications:

  • High school diploma or equivalent required; associate or bachelor's degree in healthcare, social services, public health, or a related field preferred.
  • Previous experience in patient outreach, care coordination, healthcare navigation, medical assisting, or a call-center environment preferred.
  • Experience working with electronic health records
  • Strong verbal and written communication skills.
  • Ability to communicate calmly and professionally with patients experiencing health concerns.
  • Strong attention to detail and ability to maintain complete, accurate documentation.
  • Ability to manage multiple patient tasks, alerts, and follow-up items efficiently.
  • Comfortable learning and working in multiple healthcare technology platforms.
  • Ability to recognize when a concern requires escalation and follow established clinical protocols.

Schedule: Monday–Friday, -9:00 a.m.–6:00 p.m. CST; remote position

Weekends: None

Salary:USD 1,000–1,200 per month, depending on experience.
