About the RoleWe are seeking an experienced Virtual Registered Nurse (RN) with a strong background in home care to support our healthcare clients remotely. This role is ideal for an RN who is highly organized, tech-savvy, and comfortable coordinating patient care, documentation, and communication in a virtual environment.
You will work closely with home care agencies, caregivers, and patients to ensure continuity of care, compliance, and high-quality clinical supportwithout the need for bedside duties.
Key Responsibilities- Conduct virtual patient assessments and follow-ups
- Review, coordinate, and update care plans in collaboration with physicians and caregivers
- Perform OASIS review and clinical documentation (as applicable)
- Monitor patient progress, identify risks, and escalate concerns appropriately
- Coordinate with interdisciplinary teams (caregivers, schedulers, clinicians)
- Ensure compliance with home care regulations and agency policies
- Maintain accurate and timely EMR documentation
- Provide clinical guidance and support to non-clinical staff as needed
Qualifications- Active Registered Nurse (RN) license (US and/or PH RN)
- Minimum 23 years of home care or home health experience
- Prior experience working in a virtual or remote healthcare role is highly preferred
- Strong knowledge of home care workflows, regulations, and documentation
- Proficiency in EMRs (Homecare Homebase, WellSky, Kinnser, or similar systems is a plus)
- Excellent written and verbal English communication skills
- Reliable high-speed internet and a quiet home office setup
