Registered Nurse (RN) who is passionate about empowering patients with chronic conditions to live healthier, fuller lives.
In this fully remote role, you'll be a vital lifeline—connecting with patients, guiding them in managing their conditions, and making a real impact on their health outcomes.
What You'll Do
- Connect & Care: Make monthly outreach calls to patients enrolled in our CCM program.
- Personalized Care Plans: Collaborate with patients and providers to design and update individualized care strategies.
- Early Intervention: Monitor symptoms, identify risks, and escalate care before complications arise.
- Coordinate Care: Work seamlessly with primary care providers, specialists, and community resources.
- Patient Education: Equip patients and caregivers with knowledge on disease management, medication adherence, and healthy habits.
- Document with Precision: Maintain accurate and timely patient records in our EHR system.
- Stay Compliant: Follow CMS guidelines for CCM while promoting preventive care and lifestyle improvements.
What You Bring
- Valid RN license in the Philippines
- Minimum of 1 year of clinical nursing experience (chronic care, case management, or primary care experience is a plus)
- Strong knowledge of chronic conditions such as diabetes, COPD, hypertension, and heart failure
- Excellent communication, critical thinking, and organizational skills
- Comfort using EHR systems and virtual care tools
- Ability to work independently while thriving in a collaborative team environment
Bonus Points If You Have
- Familiarity with Medicare Chronic Care Management documentation and compliance
- Experience with motivational interviewing or other patient engagement techniques
