We are seeking a highly specialized Radiation Oncology Coder to support accurate, compliant coding across the full continuum of radiation oncology services—from simulation and treatment planning through delivery and advanced procedures.
This is a coding-first role requiring deep expertise in radiation oncology workflows, strong command of CPT, ICD-10, and HCPCS coding, and the ability to translate complex clinical documentation into precise, audit-ready codes. The ideal candidate understands the clinical context behind each service and applies current ASTRO and ACR guidance to ensure compliant and optimized reimbursement.
Key Responsibilities
Specialty Coding Execution
- Assign accurate CPT, ICD-10, and HCPCS codes across radiation oncology services to ensure compliant billing
- Code treatment planning, simulation, delivery, and special procedures with full understanding of clinical workflows
- Apply appropriate modifiers (e.g., TC, 26, and treatment-specific modifiers) to support correct reimbursement
Documentation Review & Charge Capture
- Review ARIA EMR documentation, treatment records, and physician notes to capture all billable services
- Identify undercoding, overcoding, and missed charges to optimize revenue integrity
- Query providers when documentation is insufficient or unclear to ensure coding accuracy
Compliance & Regulatory Alignment
- Ensure coding aligns with Medicare LCD/NCD policies, CMS guidelines, and payer-specific requirements
- Apply ASTRO and ACR coding guidance to daily coding decisions
- Stay current on annual CPT, ICD-10, and CMS updates impacting radiation oncology
Denials & Revenue Support
- Partner with billing teams to resolve coding-related denials and support appeals
- Analyze denial trends and recommend corrective actions to improve coding accuracy
- Support clean claim submission through precise and compliant coding practices
Collaboration & Clinical Alignment
- Work closely with radiation oncologists, dosimetrists, and physicists to validate documentation and services
- Provide coding guidance to clinical and billing teams to improve documentation quality
Audit & Documentation
- Participate in internal coding audits and contribute to continuous improvement initiatives
- Document coding rationale, decisions, and query outcomes within ARIA and practice systems
Qualifications
- 3+ years of hands-on radiation oncology coding experience (general oncology coding alone is not sufficient)
- Strong expertise in radiation oncology CPT code families and associated clinical workflows
- Proficiency in ARIA EMR, including treatment documentation and charge capture navigation
- Deep understanding of professional vs. technical component coding and global billing structures
- Strong knowledge of bundling rules, modifier usage, and payer-specific coding requirements
- Familiarity with ASTRO and ACR coding guidance and real-world application
- High attention to detail with strong analytical and problem-solving skills
- Excellent communication skills for provider queries and cross-functional collaboration
- Radiation Oncology Coding Certificate (ROCC) strongly preferred
- CPC, CCS, or other AAPC/AHIMA certification
- Experience coding SBRT, SRS, and brachytherapy services
- Background in both hospital outpatient and professional/physician coding
- Experience with physics and dosimetry charge coding
- Participation in ASTRO or ACR coding education or workshops
