The Assistant Manager of Public/Social Welfare Department (PSW) is responsible for planning, organizing, and managing the Department (PSW) services in both Bishan & Bedok Home. This role ensures that residents receive comprehensive psychosocial support, discharge planning, and advocacy services, while maintaining compliance with regulatory requirements and a strong partnership with the Finance Department. He/She also provides supervision and professional guidance to a small team of staff members.
Responsibilities:
- Supervise, mentor, and evaluate the performance of staff members
- Ensure department compliance with local regulations (e.g., Nursing Home Regulations)
- Oversee psychosocial assessments for all residents upon admission and periodically thereafter
- Oversee safe and effective discharge planning processes
- Monitor and ensure that PSW Executives follow up on all cases that have defaulted in monthly payments
- Ensure development and implementation of individualized care plans
- Address residents emotional, social, and psychological needs
- Intervene in cases involving crisis, abuse, neglect, or family conflict
- Conduct home visits if the resident can be discharged, and coordinate services in the community that can provide support for residents and NOKs
- Coordinate transitions to hospitals, homecare, or other facilities
- Address complaints, concerns, and grievances in a professional manner
- Develop programs to enhance residents psychosocial well-being
- Monitor quality indicators and implement improvements
- Maintain relationships with community agencies and resources
- Coordinate referrals for additional services (e.g., mental health, legal aid, financial assistance)
- Ensure proper documentation and communication with external providers
- Participate in interdisciplinary team meetings
- Facilitate family meetings and care conferences
- Provide inputs at Medifund and FAS Committee Meetings
- Provide inputs at Residents Financial Welfare Committee Meetings.
Requirements:
- Bachelor's or Master's degree in Social Work (MSW preferred)
- Current licensure or certification as required by local regulations
- Minimum 3 years of relevant experience
- Prior supervisory or leadership experience preferred
- Excellent communication and conflict resolution skills
- Ability to manage multiple priorities in a fast-paced environment
- A good team player with initiative
