Job Description

  • Assumes the duties and responsibilities of Pharmacist IV in his/her absence.
  • Supervise and organize work flow at the inpatient and outpatient services.
  • Dispenses drugs and medicine, and other pharmaceutical preparations;
  • Checks for accuracy of doses prepared:
  • Intravenous admixtures
  • Extemporaneous compounding
  • Unit dose Reviews and interprets each unit dose and IV admixture medication order to insure that it is entered accurately into the unit-dose or IV admixture system.
  • Participates in the regular meetings of the Therapeutics Committee and other hospital committees involving the Pharmacy.
  • Provides drug information to physicians, nurses, and other health-care personnel.
  • Counsels patients on medications to be self-administered in the hospital and discharge medications.
  • Monitors adverse drug experience reporting in the hospital.
  • Supervise the daily inventory of drugs and medical devices in the dispensing area and Satellite Pharmacies.
  • Monitors drugs at critical level in assigned shelves.
  • Checks incoming drugs, medicine, chemicals for quality, qualifying in conformity with requested item.
  • Monitors proper record keeping and billing:
  • Patient-medication records
  • Extemporaneous compounding records
  • Intravenous admixture and unit dose dispensing records and billing
  • Outpatient billing
  • Reports (e.g., emergency purchase report, consignment issues, etc.)
  • Prescription files
  • Records prohibited and regulated drugs issued daily.
  • Monitors recording of inpatients issues to wards covered by unit dosing in individual patients profile.
  • Monitors ledgers of patients with outside-funding.
  • Monitors correct quantity, expiry date, and proper storage of emergency drugs in the wards and clinics.
  • Monitors the procurement and recording of emergency purchase of drugs.
  • Reviews monthly reports prior to submission.
  • Monitor the filing, storage, and retrieval of pharmacy documents.
  • Prepares the schedule of duties of the dispensing area staff.
  • Prepares performance evaluation report for pharmacy staff.
  • Monitors the encoding of daily sales and inpatient issues.
  • Monitors maintenance/use of all equipment, electronic devices, accountable, and official forms.
  • Supervises clerks, Pharmacy Aides, and pharmacy interns.
  • Maintains cleanliness and physical order in the respective working areas.
  • Performs other related duties as may be assigned from time to time.
  • Compliance to IMS (Integrated Management System) Standards and Policies.
