Job Description

  • Coordinates the overall pharmaceutical needs of the patient care areas with the dispensing area.
  • Responsible and accountable in managing the medication therapy of patients.
  • Provides medication therapy evaluations and recommendations to patients and health care professionals
  • Provides patient education and medication counselling.
  • Counsels patients on medications to be self-administered in the hospital and discharge medications
  • Reviews the home medicines of discharged patients.
  • Provides drug information to physicians, nurses, and other health-care personnel.
  • Attends to adverse drug reaction experience (ADE) in the hospital.
  • Compounds parenteral, oral, and topical preparations (IV admixtures and Total Parenteral Nutrition for inpatients).
  • Reviews and interprets each unit dose and IV admixture medication order to ensure that it is entered accurately into the unit-dose or IV admixture system
  • Insures that good manufacturing techniques are used in compounding intravenous admixtures and extemporaneous preparations.
  • Provides for proper drug control:
  • Insures that drugs are stored and dispensed properly.
  • Insures that all national laws and DDB regulations are followed.
  • Dispenses drugs and medicine, and other pharmaceutical preparations
  • Daily inventory of drugs and medical devices.
  • Monitors drugs at critical level in assigned shelves.
  • Checks incoming drugs, medicine, chemicals for quality, qualifying in conformity with requested item.
  • Records inpatients issues of wards covered by unit dosing in individual patients profile.
  • Monitors correct quantity, expiry date, and proper storage of emergency drugs in the wards and clinics.
  • Monitors maintenance/use of all equipment, electronic devices, accountable, and official forms.
  • Participates in the regular meetings of the Therapeutics Committee and other hospital committees involving the Pharmacy.
  • Communicates with all Pharmacy staff regarding new developments in the area.
  • Maintains professional competence, particularly in knowledge of drug stability and incompatibilities.
  • Attend meetings/seminar-workshops/consultative meetings related to clinical and hospital pharmacy practice.
  • Supervises clerks, Pharmacy Technicians, and pharmacy interns.
  • Compliance to IMS (Integrated Management System) Standards and Policies.
