JOB DESCRIPTION Responsible for leading, managing, and monitoring the implementation of social case management interventions and social service delivery for Pantawid beneficiaries through appropriate case management approaches, referral mechanisms, convergence strategies. Ensures that households receive necessary support services toward improved well-being, and eventual graduation from the Program and follow-through services at the local government through an enhanced referral system and convergent actions and mechanisms. | |||
Functions and Responsibilities: | |||
1. Oversee the day-to-day operations of the Social Case Management Division. 2. Ensure the effective management, coordination, and monitoring of social services delivery interventions. 3. Oversee the conduct of social case management monitoring, review, and evaluation. 4. Oversee caseload inventory management and the implementation of beneficiary transition and graduation processes. 5. Lead the development and implementation of strategies and mechanisms that promote secure, sustainable and resilient transition pathways for graduating and exiting households. 6. Ensure the development, enhancement, and utilization of information systems and databases for monitoring graduating households. 7. Lead the formulation, revision and enhancement of policies, guidelines, manuals and related issuances to support alignment with the Management's goals, emerging directives, and organizational mandates. 8. Coordinate responses and provide technical assistance to NPMO Divisions, other OBSUs within DSWD, and other entities to the implementation concerns in relation to social services delivery and management. 9. Submit regular, periodic, and special reports to the National Program Manager, including recommended actions and interventions for urgent issues affecting social services delivery and case management implementation. 10. Attend management meetings, technical consultations, and other official engagements, and designate an authorized personnel/focal person when necessary. 11. Perform other necessary tasks that may be assigned by the National Program Manager. | |||
