What is the job
The Patient Care Coordinator is responsible in providing clinical and administrative support to a U.S.-based outpatient healthcare practice by conducting patient outreach, pre-visit intake, medical documentation, referral coordination, and follow-up activities. The role works closely with physicians and clinical staff to ensure accurate patient information, timely documentation, and coordinated patient care while maintaining confidentiality and compliance with HIPAA requirements.
Responsibilities:
- Conduct outbound pre-visit calls to gather and update relevant patient information before scheduled appointments.
- Handle inbound patient inquiries and follow-up calls within established practice protocols.
- Gather and document Chief Complaint (CC), History of Present Illness (HPI), patient-reported symptoms, medication information, and relevant medical history.
- Review previous visit notes and obtain updated information from patients.
- Review medication lists and document patient-reported updates.
- Identify incomplete or missing information and follow established workflows to obtain required documentation.
- Escalate clinical questions, concerns, or information requiring provider or licensed clinical judgment to the appropriate staff.
- Prepare patient charts and visit documentation before provider appointments.
- Accurately document patient information and other clinical details in the EHR/EMR.
- Support providers with real-time medical scribing and clinical documentation when required.
- Review charts for completeness and ensure relevant patient information is available to providers.
- Maintain timely, accurate, and professional documentation in accordance with practice standards.
- Support approximately 20–25 patient encounters per day while maintaining documentation quality and accuracy.
- Prepare and submit referral requests according to established practice workflows.
- Follow up on referral approvals, appointments, and status updates.
- Assist with coordination and follow-up of laboratory orders and diagnostic testing.
- Communicate with patients regarding referrals, testing, and other follow-up activities.
- Coordinate with physicians, healthcare facilities, laboratories, and other stakeholders as needed.
- Document referral and care coordination activities accurately in the patient's medical record.
- Escalate authorization, referral, or clinical concerns to the appropriate staff.
- Schedule and coordinate patient-related activities as assigned.
- Communicate professionally and compassionately with patients, providers, healthcare facilities, and other stakeholders.
- Handle inbound and outbound patient communications.
- Support provider efficiency through proactive chart preparation and follow-up.
- Maintain HIPAA compliance, patient confidentiality, and appropriate handling of protected health information.
- Follow established practice protocols, workflows, and escalation procedures.
- Perform other related clinical administrative duties as assigned.
Qualifications:
- Minimum of two (2) years of experience in customer service or a related support role.
- Comfortable handling customer interactions through phone support when needed.
- Demonstrates strong attention to detail and ability to maintain accurate customer information.
- Demonstrates strong problem-solving skills and ability to handle customer concerns professionally.
- Proven ability to learn and navigate multiple software platforms, CRM systems, messaging tools, and reservation workflows.
- Proven ability to work independently while maintaining productivity in a remote work environment.
- Clear and coherent both written and verbal communication skills in English.
- Must have stable employment history.
