Job Description

We are seeking an experienced Ophthalmology Medical Scribe with hands-on experience using ModMed EMA to provide real-time clinical documentation support to ophthalmologists.

The ideal candidate understands the flow of an ophthalmology clinic, is highly familiar with ophthalmic terminology and diagnostic testing, and can accurately document physician-patient encounters in real time. This role is critical in helping physicians maintain efficient patient flow while ensuring clinical documentation is complete, accurate, and ready for physician review and signature.

Key Responsibilities

Pre-Visit Chart Preparation

  • Review the daily patient schedule and prepare charts before appointments.
  • Review prior ophthalmology notes, diagnoses, medications, surgical history, and relevant diagnostic testing.
  • Identify pertinent clinical information for the physician's review.
  • Prepare appropriate ModMed EMA templates and workflows for each encounter.

Real-Time Clinical Documentation

  • Document physician-patient encounters accurately and efficiently in ModMed EMA.
  • Capture key clinical information, including:
    • Chief complaint and HPI
    • Ocular and medical history
    • Examination findings
    • Assessment and diagnoses
    • Treatment plans
    • Medications
    • Follow-up instructions
    • Physician interpretations of diagnostic testing
  • Maintain accurate OD/OS/OU laterality throughout documentation.
  • Accurately capture physician instructions regarding medications, procedures, testing, referrals, and follow-up.
  • Adapt documentation style and workflow based on individual physician preferences.
Ophthalmology Documentation

  • Demonstrate familiarity with common ophthalmic conditions, including:
    • Glaucoma
    • Cataracts
    • Diabetic retinopathy
    • Macular degeneration
    • Dry eye
    • Retinal conditions
    • Corneal disease
    • Postoperative care
  • Understand and accurately document common ophthalmic diagnostic testing, including:
    • OCT
    • Visual fields
    • Fundus photography
    • Pachymetry
    • Topography
    • Refraction
    • IOP
    • Slit-lamp examination
    • Dilated fundus examination
  • Support documentation for new patient, follow-up, postoperative, procedure, and testing-only visits as applicable.
EMR & Workflow Management

  • Navigate ModMed EMA efficiently and accurately.
  • Use established templates, protocols, and workflows to maximize documentation efficiency.
  • Review completed notes for accuracy and completeness before submitting them for physician review.
  • Identify unclear or incomplete information and appropriately flag it for the physician.
  • Keep documentation current while supporting efficient patient flow throughout the clinic day.

Compliance & Quality

  • Maintain strict HIPAA compliance and patient confidentiality at all times.
  • Follow EMR security protocols and practice-specific policies.
  • Use individual system credentials appropriately.
  • Never independently modify clinical information or make clinical decisions.
  • Ensure all documentation accurately reflects the physician's findings and instructions.
  • Ensure clinical documentation is reviewed and approved by the physician before finalization.

Qualifications

  • 2+ years of medical scribe experience, preferably in ophthalmology.
  • Hands-on experience using ModMed EMA is required.
  • Previous experience documenting ophthalmology encounters in ModMed.
  • Strong understanding of ophthalmology terminology and clinical workflows.
  • Excellent English communication, grammar, spelling, and medical terminology skills.
  • Ability to accurately listen, interpret, and document conversations in real time.
  • Exceptional attention to detail, particularly with:
    • Diagnoses
    • Medications
    • Laterality
    • Diagnostic testing
    • Physician instructions
  • Strong computer and multitasking skills.
  • Ability to work independently with minimal supervision.
  • Ability to maintain both speed and accuracy in a high-volume clinical environment.
  • Previous experience working directly with an ophthalmologist.
  • Experience supporting multiple ophthalmology subspecialties, including:
    • Retina
    • Glaucoma
    • Cornea
    • Cataract
    • Comprehensive Ophthalmology
    • Oculoplastics
    • Refractive/LASIK
  • Experience with ophthalmology-specific ModMed EMA templates and workflows.
  • Experience documenting OCT, visual fields, fundus photography, and other ophthalmic diagnostic testing.
  • Certified Ophthalmic Scribe (OSC) or similar certification is a plus.
