We are looking for a licensed and compassionate Offshore Nurse to support clinical triage and care coordination for U.S.-based healthcare operations. This role is ideal for someone experienced in patient communication, discharge planning, and remote clinical support.
Key Responsibilities:
- Perform clinical triage using provided tools (e.g., ClearTriage)
- Handle inbound calls from hospitals requesting assistance with patient discharge planning
- Coordinate transitions of care to Skilled Nursing Facilities (SNF) or home health services
- Accurately document patient information and case details
- Communicate effectively with healthcare providers and internal teams
- Ensure timely handling of requests while maintaining quality and compliance
What We're Looking For:
- Active nursing license in at least one of the following U.S. states:
- Texas (TX)
- Arizona (AZ)
- Nevada (NV)
- California (CA)
- Utah (UT)
- Experience in clinical triage, case management, or discharge planning
- Strong phone handling and communication skills
- Ability to manage multiple cases in a fast-paced environment
- High attention to detail and documentation accuracy
- Comfortable working remotely during night shift hours
- Experience with healthcare systems or triage tools is a plus
