About the Role

We are seeking an experienced Medical Transcriptionist/Medical Scribe with strong clinical documentation and chart preparation experience. This role supports a physician before, during, and after patient encounters. It is primarily a pre-visit clinical documentation position with ongoing note updates—not a traditional transcription-only role. The physician remains responsible for verifying clinical information, approving notes, and signing final documentation.

Key Responsibilities

Pre-Visit Preparation

• Review the patient's available medical records in Epic.
• Gather relevant history and physical reports, consultation notes, laboratory results, imaging reports, and previous documentation.
• Organize relevant clinical information into a structured summary.
• Use OpenEvidence or the hospital's approved AI tool, as directed by the physician, to assist with preparing summaries and draft notes.
• Prepare the patient's chart and draft documentation before the physician sees the patient.

During the Encounter

• Make the prepared chart and summary available to the physician.
• Receive instructions from the doctor regarding changes to the documentation.
• Update draft notes with information the physician provides.
• Document orders the physician requests, within the permissions granted by the hospital.

After the Encounter

• Modify clinical notes according to the physician's instructions.
• Incorporate corrections and additional information provided by the doctor.
• Prepare notes for final review.
• Leave all documentation ready for the physician to review and sign at the end of the day.

Requirements

Non-Negotiables (Must Have)

• Extensive medical scribing and clinical documentation experience.
• Previous experience working as a medical scribe.
• Ability to prepare structured clinical notes.
• Experience reviewing clinical records and compiling information into documentation for physicians.
• Ability to update medical notes accurately based on physician instructions.
• Must require minimal training in standard scribing responsibilities.
• Strong clinical knowledge and chart preparation experience.
• Ability to understand medical terminology.
• Experience reviewing history and physical reports, specialist consultations, laboratory findings, and imaging reports.
• Ability to summarize relevant patient information accurately.
• Familiarity with clinical documentation workflows and physician note preparation.
• Experience working with electronic health records.
• Prior experience documenting patient encounters within an EHR.
• Ability to review existing medical records and prepare draft notes.
• Familiarity with EHR documentation workflows.
• Ability to work accurately with patient information.
• Must be amenable to work onsite at Jollibee Tower, Ortigas.

Preferred (Negotiables)

• Direct experience with Epic.
• Previous medical scribing experience in a hospital setting.
• Experience preparing clinical summaries using approved AI documentation tools.
