Medical Records Specialist Urology | Permanent WFH

We are seeking a detail-oriented Medical Records Specialist – Urology to support a urology practice with medical record management, chart preparation, and risk adjustment documentation review.

Key Responsibilities

• Prepare and organize patient charts before scheduled appointments, ensuring all necessary medical records and clinical documentation are available.
• Review patient records to identify missing, incomplete, inconsistent, or relevant clinical information.
• Review historical medical records to identify relevant diagnoses and supporting documentation for risk adjustment preparation.
• Organize clinical documentation to support accurate diagnosis capture and appropriate risk adjustment workflows.
• Navigate the ModMed EMR to retrieve, review, and maintain patient information efficiently.
• Identify documentation gaps and discrepancies, escalating issues to the appropriate clinical or administrative personnel.
• Ensure medical records are accurate, complete, properly organized, and accessible for provider review.
• Maintain patient confidentiality and comply with HIPAA regulations and applicable medical records policies.
• Collaborate with clinical and administrative teams to support timely chart preparation and efficient practice operations.
• Manage multiple records and preparation tasks while maintaining accuracy and meeting established deadlines.
  
  

Qualifications

• Previous experience in medical records management, chart preparation, medical record review, or a similar healthcare role.
• Experience with risk adjustment documentation preparation, diagnosis identification, or diagnosis capture workflows is strongly preferred.
• Familiarity with ModMed EMR is an advantage.
• Strong understanding of medical terminology and clinical documentation.
• Ability to review patient histories and identify relevant diagnoses and supporting documentation.
• Excellent attention to detail, organizational skills, and commitment to data accuracy.
• Ability to manage high volumes of medical records while maintaining quality and efficiency.
• Strong written and verbal communication skills.
• Ability to work independently, prioritize tasks, and meet deadlines.
• Experience supporting a urology practice is a plus.
• Amenable to work during US hours
• Must be willing to undergo a background check as part of the hiring process.
