Home Health Case Manager

Omni Therapy, Inc. is looking for dedicated, knowledgeable Case Managers to handle home health referrals on our team. Candidates should have successful experience working in the healthcare field as a healthcare case manager or care coordinator.

Case Managers work side-by-side with members of the staffing, Q.A. and human resource teams to provide support by managing activity on referrals after the initial visit has already been conducted by a therapist.

Job Requirements:

• 2+ years of recent experience as a healthcare care coordinator / healthcare case manager

• Experience handling PT / OT / ST charts is a big plus.

• Must be available to work full-time hours: Mon – Fri, 9am – 5:30pm Pacific Standard Time.
