Job title: Remote Registered Nurse – Home Care Caregiver Support & Accountability
Type of employment: Full-time
Shift Schedule: 3.5 days per week, covering long daytime New York shifts. Expected coverage is approximately 7:00 AM to 8:00 PM ET for full shift days, with one shorter half-day shift. Final A/B shift assignment to be confirmed by the client.
We are seeking a proactive and clinically experienced Remote Registered Nurse to support a New York-based home care agency's Patient Care team. This role will work closely with the agency's NYC-based Registered Nurses, Patient Care Coordinator, and caregiver team to help monitor active caregiver shifts, support care-plan follow-through, and maintain high standards of care for senior clients receiving in-home support.
The main focus is caregiver support, task follow-up, clinical judgment, documentation, and real-time accountability through WhatsApp and phone communication.

The client is a New York City-based home care agency providing in-home care for seniors through home health aides. The agency supports approximately 120 clients and around 250 caregivers. Their care model focuses on providing real-time support to caregivers while creating stronger accountability around care-plan execution, client safety, and service quality.

Objective of this role:
- Support caregivers during active shifts, help maintain care-plan compliance, and provide real-time communication support to the Patient Care team so clients receive consistent, high-quality home care.
Responsibilities:
Caregiver Shift Support & Accountability
- Monitor active caregiver shifts through WhatsApp and phone communication.
- Remind caregivers to complete scheduled care-related tasks such as medications, vitals, meals, hygiene support, mobility assistance, and other care-plan items.
- Request and review caregiver updates, photos, videos, or confirmations when required by the care plan or client consent.
- Follow up when caregivers miss required updates or do not complete expected documentation.
- Help verify that caregivers are following assigned care plans and agency expectations.
- Provide guidance and reminders in a professional, respectful, and supportive manner.
High-Acuity Client Support
- Support higher-acuity client cases under the direction of the NYC-based Registered Nurses.
- Use nursing judgment to understand which clients, caregivers, and shift tasks require closer follow-up.
- Prioritize urgent or time-sensitive care needs across multiple active shifts.
- Escalate clinical or care-related concerns to the appropriate NYC nurse or Patient Care team member.
- Help identify possible changes in client condition based on caregiver reports, photos, videos, or shift updates.
Communication & Relationship Management
- Build strong working relationships with caregivers while maintaining clear accountability.
- Communicate with caregivers through WhatsApp, phone, and other internal systems.
- Balance a supportive tone with the confidence to follow up firmly when tasks need to be completed.
- Coordinate with the Patient Care Coordinator, NYC-based RNs, and internal teams regarding shift issues, caregiver concerns, and client updates.
- Document follow-ups clearly and keep communication organized.
Documentation & Follow-Up
- Assist the Patient Care Coordinator with care-related follow-up and documentation.
- Track caregiver updates, shift concerns, and outstanding tasks.
- Maintain accurate notes on care-plan follow-through, caregiver compliance, and escalated issues.
- Support internal visibility around client care, caregiver performance, and task completion.
Required skills and qualifications:
- Registered Nurse with hands-on nursing experience; nursing education alone is not sufficient.
- Experience in home care, senior care, patient coordination, clinical support, healthcare operations, or a related nursing environment.
- Strong English communication skills, both spoken and written.
- Confident phone and WhatsApp communication skills.
- Ability to manage multiple active shifts, tasks, and caregiver conversations at once.
- Strong prioritization and task management skills.
- Able to work independently in a remote setup with minimal hand-holding.
- Comfortable holding caregivers accountable while maintaining positive working relationships.
- Able to escalate concerns appropriately and communicate clearly with internal teams.
- Comfortable working New York daytime hours from the Philippines.
Preferred skills and qualifications
- Remote healthcare or remote home care experience.
- Experience supporting caregivers, home health aides, nurses, or field staff.
- Experience working with high-acuity patients or senior care cases.
- Familiarity with U.S. home care, patient care coordination, or care-plan documentation.
- Experience using WhatsApp, phone systems, EMR-like systems, Google Sheets, or CRM-style trackers.
- Prior experience working with U.S.-based healthcare teams.
