Experience: Skills in conducting patient outreach and welcome calls, completing demographic and insurance verification, and performing program eligibility screening.

Responsibilities:
- Conduct timely outbound calls to new patients to welcome them to the program or service line. - Provide clear information about the onboarding process, next steps, required documentation, and appointment details.
- Confirm and update patient demographic details such as name, address, contact information, date of birth, etc.
- Ensure all data is accurately entered into the EHR/EMR or CRM platform.
- Verify insurance eligibility, benefits, and coverage for scheduled services.
- Identify insurance discrepancies and coordinate with patients to obtain correct information. - Conduct screening assessments based on program-specific guidelines (clinical, financial, or geographic).
- Confirm that patients meet necessary qualification criteria prior to enrollment.
- Escalate complex eligibility issues to supervisors or program coordinators.
- Maintain strict adherence to compliance, HIPAA, and payer requirements.
