Job Responsibilities
- Telephone Triage & Health Assessment
Assess patient symptoms and medical concerns over the phone.
Use clinical judgment and established protocols to determine the level of care needed.
Provide health education, self-care advice, or escalate to appropriate healthcare services.
- Patient Support & Education
Educate patients about medications, chronic disease management, preventive care, and follow-up procedures.
Provide information on healthcare services, benefits, and available programs.
- Case Management Assistance
Assist with coordination of care for patients with chronic illnesses or post-hospitalization.
Follow up on patient progress and ensure adherence to treatment plans.
College Graduate
PHRN/RPH with active license
Open to newly registered Pharma and PHRN
Open to those with no bedside experience
Night shift , weekends off but candidate should be amenable w/ scheduled weekend shift if needed
Onsite deployment (SH4) for 6 months then will shift to hybrid (3x a week onsite)
