Job Description

Job Summary We are seeking an experienced and detail-oriented Certified Medical Coder with strong EPIC EMRexperience and a background in Primary Care and/or Urgent Care coding to join our healthcare team. This role is responsible for reviewing clinical documentation, assigning accurate diagnosis and procedure codes, ensuring coding compliance, and supporting optimal reimbursement while maintaining adherence to payer and regulatory guidelines.

Job Responsibilities

  • Review clinical documentation and accurately assign ICD-10-CM, CPT, and HCPCS codes for Primary Care and Urgent Care encounters.
  • Validate E&M service levels based on current coding guidelines and documentation standards.
  • Perform coding audits to ensure documentation supports billed services and identify opportunities for improvement.
  • Identify and resolve coding discrepancies, missing documentation, and claim edits.
  • Collaborate with providers, clinical staff, and revenue cycle teams to improve documentation quality and coding accuracy.
  • Monitor coding denials and provide recommendations to reduce future denials and improve reimbursement.
  • Stay current with updates to CPT, ICD-10, HCPCS, CMS, and payer-specific coding guidelines.
  • Maintain compliance with federal regulations, payer requirements, and organizational policies.
  • Provide coding-related education and feedback to providers and staff as needed.
  • Support revenue cycle initiatives aimed at improving clean claim rates and overall financial performance.

Qualifications

  • 5+ years of medical coding experience.
  • Experience conducting coding audits and providing provider education.
  • Familiarity with Medicare, Medicaid, and commercial payer policies.
  • Multi-specialty outpatient coding experience.
  • Current coding certification such as CPC (Certified Professional Coder), CCS-P (Certified Coding Specialist – Physician-Based), or COC (Certified Outpatient Coder).
  • Minimum of 3 years of professional medical coding experience.
  • Hands-on experience with EPIC EMR.
  • Strong knowledge of ICD-10-CM, CPT, HCPCS, and E&M coding guidelines.
  • Experience coding for Primary Care and/or Urgent Care services.
  • Understanding of payer guidelines, medical necessity requirements, and coding compliance standards.
  • Excellent attention to detail, analytical abilities, and problem-solving skills.
  • Strong written and verbal communication skills.
