Role Description This is a full-time remote Case Manager role responsible for supporting clients as they work toward their recovery and wellness goals. Day-to-day tasks include conducting intake assessments, developing individualized service plans, coordinating referrals to appropriate community and clinical resources, and monitoring progress through regular check-ins. The Case Manager will maintain accurate documentation, track outcomes, and ensure continuity of care by collaborating with healthcare providers, social service agencies, and support networks. The role also involves advocating for clients, addressing barriers to care such as housing or transportation, and providing education on available services and recovery options. The Case Manager will use secure digital tools to communicate with clients, manage caseloads, and participate in virtual team meetings and case reviews.
Qualifications
- Demonstrated skills in case management, care coordination, and client advocacy, with the ability to develop and follow individualized service plans.
- Strong communication, active listening, and interpersonal skills, including the ability to build trust with clients and collaborate effectively with multidisciplinary teams.
- Organizational and time-management skills, including maintaining detailed records, managing multiple cases, and meeting documentation and reporting requirements.
- Comfort working remotely with digital tools for client interaction, data entry, and secure information sharing; proficiency with basic office software and telehealth platforms.
- Knowledge of mental health, substance use, or social services systems and familiarity with community resources that support recovery and stability.
- Ability to work with people from diverse backgrounds in a non-judgmental, trauma-informed, and culturally responsive manner.
- Bachelor's degree in social work, psychology, counseling, human services, or a related field; relevant certifications or licensure are an advantage.
- Previous experience in case management, social services, or behavioral health settings is strongly preferred.
