About the Role
We are looking for a dedicated Care Coordinator with a strong nursing background and clinical experience to join our healthcare team. The ideal candidate has hospital or clinical experience and preferably has worked in case management or transition of care. This role requires working onsite at our Bacolod office and on a night shift schedule.
Responsibilities
- Manage and coordinate chronic care plans for patients with long-term health conditions.
- Monitor and assess patient conditions to ensure proper care and smooth transitions between care settings.
- Collaborate with healthcare providers and case managers to facilitate seamless care transitions.
- Prepare and process clinical reports and documentation related to chronic care management.
- Follow protocols and apply clinical guidelines in patient care.
- Provide education and support to patients and their families regarding chronic condition management.
Qualifications
- Graduate of BS Nursing with PHRN license
- Active USRN license for TEXAS
- At least 1-3 years of hospital or clinical experience (tertiary hospital)
- Previous BPO experience is an advantage, not required
- Preferably with experience in Case Management or Transition of Care.
- Open to BS Nursing Graduates (non-licensed) with prior BPO experience in a Healthcare account.
- Willingness to work onsite in Bacolod and on a night shift schedule.

Pay range and compensation package
- 15% Night Differential
- HMO coverage after 30 days of employment (with option to add up to 2 dependents)
