Role Purpose The Care Coordinator supports older adults receiving Home Care Package and Support at Home services by managing onboarding, service coordination, scheduling, referrals, documentation, and client communication. The role ensures services are safe, goal-aligned, culturally inclusive, and responsive to each client's needs. It also monitors wellbeing, identifies early deterioration, assists with risk assessments, and supports care plan reviews in line with wellness, reablement, and dignity-of-risk princip
