Position Summary:
This position is responsible for coordinating with the Program Manager to ensure client engagement and measurement data is tracked in accordance with the needs of the program and its anticipated goals. The Coordinator supports the Substance Use Disorders (SUDS) program by ensuring tasks are completed, and clients have been contacted and educated about the available services.
Position Responsibilities include, but are not limited to the following:
1. Perform outreach to the provided target population list to educate clients on services offered for substance use disorder services and programs daily.
2. Link clients to appropriate SUDS services while monitoring Provider schedules and ensure they are up-to-date and maximized. Additionally, engaging clients in services needed such as IOP, Therapy, and Psychiatry, as appropriate.
3. Ongoing management of calls where the caller is identified as in crisis, as soon as possible, to ensure safe outcomes whenever possible.
4. Coordination of Care for non-intervention population of SUDS participants
5. Collect and enter data relating to Inputting all measurements and data points needed for internal and external reporting.
Knowledge, Skills, and Abilities preferred:
. Experience working with behavioral health programs, counseling, and/or mental health services as well as those treating Substance Use Disorders, for a minimum of two (2) years, (recommended).
. Strong organizational and leadership skills ability to multitask, to set and follow priorities, and ensure focus on attention to detail.
. Ability to thrive in a fast-paced environment with flexibility to manage changes, as they occur.
. A minimum of 3+ years experience in a health-related field, (mental or medical) required.
. Knowledge and skills in managing computer-based work including the use of Excel, Word, and Google products, or the ability to learn new/cutting-edge platforms or databases.
. Excellent communication skills, ability to delegate, and manage deadlines, as indicated.
. Use of Electronic Health Record (EHR) and online databases, as well as eligibility databases, and records management systems.
. Energetic, upbeat, and positive attitude with an interest in working in an environment that expects dedication and hard work.
Education and Experience:
. Clinical experience working in a Psychiatric, Nursing or Social Work field for two (2) years is preferred, however, will consider an equal combination of education and experience.
. College Graduate, (required).
. Previous experience in healthcare administration or direct client care (preferred).
